The Discovery Tool Book
 
Another way of using the Discovery Tool is to obtain the book where all of the questionnaires for different aged children are available.  There are questionnaires for the following aged children:
Birth through 3 months
4 through 5 months
6 through 8 months
9 through 11 months
12 through 14 months
15 through 17 months
18 through 23 months
2 years through 2 years, 5 months
2 years, 6 months -- 2 years, 11 months
3 year-old children
4 year-old children
The book comes with a scoring sheet that will allow you to determine whether you should seek out professional help for a particular child.
